We installed the plugin and setup the appropriate pages. We configured an Application Submit to send an email. When testing, we don't get the email. So we installed WP Mail Log to see where the problem was. After filling out the form and getting a success message, nothing is logged in the Mail Log, though a record is added to the All Applications view.

Have you set "From email address"? also try "Reset" and save the settings.
