When i update page with this short code
[job_bm_job_submit]
I get fatal error as i mentioned above

I just checked the plugin, don't see "job_bm_job_submit_form_title" function is redeclared
can you please reinstall the plugin and let me know the result.

Also can you please check you might be used shortcode 2 more times on same page cause the issue.
